Costs and Coverage

We accept most major health care plans. However, you should confirm coverage with your insurance provider prior to your visit. A valid insurance card must be presented at each visit.

Payment (or copayment as required by health insurance plans) is due at the time of service. We accept payment in the form of all major credit cards, cash and checks.

Please Note:

At this time, we are unable to accept UnitedHealthcare’s Community Health Plan.
